Insurance & payment

Using insurance or health accounts

I am an in-network provider with Anthem/Blue Cross/Blue Shield, United Healthcare, and Medicare. For other insurers I can provide a superbill for potential out-of-network reimbursement. Payment is due at the time of service via credit card, HSA, or FSA.

Please contact your insurer to confirm mental health benefits, deductible status, and telehealth coverage prior to your first session. Coaching services are self-pay only.

Questions to ask your insurer

  • Do I have benefits for outpatient psychotherapy with CPT codes 90791 and 90837?
  • What is my deductible, copay, or coinsurance?
  • Are telehealth sessions covered when I am located in Indiana?
Good Faith Estimate

Your rights under the No Surprises Act

You have the right to receive a Good Faith Estimate (GFE) explaining how much your medical and mental health care is expected to cost. This applies to clients who are uninsured or who choose self-pay. You will receive a written GFE at least one business day before your scheduled service and can request an updated estimate at any time.

If you receive a bill that is at least $400 more than your GFE, you may dispute the bill. For questions, visit CMS No Surprises Act resources or call (800) 985-3059.
