This website collects minimal personal information. Analytics events capture anonymous usage data (e.g., page views, button clicks) to improve the experience. IP addresses are anonymized via Google Analytics 4 configuration. Cookies are not used for advertising or tracking across sites.
The contact form sends messages via email; please avoid sharing protected health information (PHI). Clinical forms and messaging occur in the secure client portal.
How health information is protected
Client records are maintained in a HIPAA-compliant electronic health record with role-based access, encryption in transit and at rest, and audit logging. Records are retained for at least seven years as required by Indiana law. You may request copies or amendments to your records in writing.
When information may be shared
Information may be disclosed with your written consent or when required by law, including suspected abuse or neglect, court orders, or imminent risk of harm to self or others. For insurance billing, necessary information may be shared with payers to obtain reimbursement.
